How to Apply for a TriTerm Medical Plan

Once you’ve made the decision to apply for TriTerm Medical insurance, the process is pretty simple. Remember, these short term insurance plans are available any time of the year so you don’t need to wait for an open enrollment period.

1. Get started

Either visit UHOne.com and complete the application steps on your own, or contact an insurance broker who is appointed with Golden Rule Insurance Company to help you apply.

2. Provide your information

Whichever method you choose, you’ll be asked to provide some basic information – name, gender, birthdate – about yourself and your family members who will be included on the application. You can then review the TriTerm Medical brochure and select a plan and deductible amount to get a quote.

3. Submit Application and Payment

When your application is ready, you can submit it electronically along with your premium payment information. The first premium payment will be collected once the application is approved. There is also a non-refundable $40 application fee in most states.

What to Expect When You Apply

TriTerm Medical plans are medically underwritten, so not all applicants will be approved. The application will include questions about tobacco use, existing coverage, medical history, prescription medications, and past treatments or diagnoses. Adults on the application will also need to provide height and weight information. If your application is approved, coverage can begin as soon as five days after the application is received.
